Having surgery is an important decision for you and that is why we require you to complete a number of forms and ask that you read the brochures provided. Here is a guide to our forms and brochures.

About our forms

Consent: Form 1

Relates to your medical diagnosis and details of the treatment which will be provided to you. You can fill in most of this information yourself but some sections should only be completed and signed after consultation with your specialist(s).

Admission: Form 2

Contains personal and payment information and your agreement to pay the hospital charges.

Health Questionnaire: Form 3

Provides us with necessary details about your medical history.

Patients may also be given a Nursing Assessment Questionnaire to complete. Please return it with your other forms or bring with you on admission.
