There is a lot for you to think about prior to coming to hospital for your surgery and that is why we strongly recommend that you read our Important Pre-Admission Information booklet that your surgeon will have given to you. It contains important information that you need to know, including what to do the week before, the day prior and the day of surgery.

Your forms

Your forms are inside the back cover of the booklet. Please complete these at your leisure but we do ask that you return them to us at least 7 - 10 working days prior to your admission.

Note: If attending an anaesthetist clinic prior to admission, please take completed Forms 1 and 3 with you. You should return Form 2 to Royston Hospital.

The forms can be delivered, posted, faxed or emailed to us.
