Checklist - Before you arrive |

- Phone the hospital to obtain cost estimates and if required discuss any special accommodation request.
- Phone your insurance company to obtain prior approval for your treatment.
- Complete and sign your Admission Forms and deliver, post, fax or email them to Royston to arrive at least 7 – 10 working days prior to your admission. If this is not possible, please bring them with you on admission. Note: If attending an anaesthetic clinic prior to your admission you will need to take completed Forms 1 and 3 with you. You should return Form 2 to Royston Hospital.
- Ensure that any laboratory tests requested by your specialist have been undertaken.
